When I talked to people afterward, almost everyone had a version of the same story: a parent, a spouse, a friend, lost in the gap between "medically ready" and safely home. This is not a rare failure. For too many families, it is the ordinary experience of leaving a hospital.
We built Talamel so that fewer families face what ours did. We spent time on hospital floors with the RN case managers who live this every day, before writing a single line of code, and built Cohezon to make the journey after a hospital stay something patients and families can actually navigate.
